• If you are located outside the Houston area, whether you would be able to temporarily relocate for the duration of a program

• If you are a caregiver or family member contacting us on behalf of a patient, your own name, phone number, and email address in addition to the patient’s details

Contact information (name, email, phone number, and zip code) is retained by our clinical team for follow-up purposes.

Health-related information shared during the conversation (your condition, time since injury, rehabilitation goals, program preferences, self-pay comfort, and relocation availability) is used solely to assess program fit. It is not stored in a structured patient record, though full conversation transcripts may be retained for quality and compliance purposes.

Caregiver contact details, where provided, are retained alongside patient contact information for follow-up purposes.

We do not collect Social Security numbers, payment card details, insurance information, or detailed medical records through this chat service.

2. How We Use Your Information

We use the information you share solely to:

• Determine whether CoLift's programs may be a good fit for you or your loved one

• Follow up with you to discuss your options

• Prioritize our clinical team's outreach to the most time-sensitive cases
